Find Non-Compliant Patients

Phytel Outreach is based on a secure Patient Registry that is developed, operated and maintained for your organization at Phytel’s SAS 70 Type II certified data center. The outreach process is driven by evidence-based protocols that reflect the most up-to-date standards of care, including published guidelines from organizations such as NCQA, HEDIS, AAFP, and NQF. In addition to standard protocol sets, Phytel can easily develop customized protocols for your organization’s specialty needs, and offers protocols for ancillary services such as bone density scans and echocardiograms. The Patient Registry is updated automatically from your Practice Management System and other data and lab systems to ensure that information on patient compliance and scheduling is always current.

Daily scans of your Phytel Patient Registry quickly and effectively target patients with lapses in recommended visits, tests, procedures or other medical services. Since the records of non-compliant patients typically reveal several non-compliant conditions, Phytel Outreach is designed to manage the status of protocols across multiple co-morbidities and preventive care criteria.

EXAMPLE
One of your patients is due for an HbA1c test, a hypertensive follow-up visit, a mammogram and a dexa scan. Phytel Outreach detects all of these non-compliant conditions, flags them for attention, and facilitates coordinated contact and scheduling.

Once Phytel Outreach has identified a non-compliant patient, the system automatically initiates a telephone call that prompts the patient to schedule an appointment. Calls are recorded in a natural, friendly voice and—subject to HIPAA confidentiality restrictions regarding diagnoses—can include specifics about the kind of visit being recommended. Standard and specialized notifications include options such as:

PREVENTIVE CARE (e.g., annual visit, well child exam)
CHRONIC DISEASE MANAGEMENT (e.g., diabetes monitoring, hypertension follow-up)
TREATMENT PLAN COMPLIANCE (e.g., lab tests)

Notification schedules are flexible and can run on any days or times that you prefer. Outreach cycles typically consist of three to five contact events over an eight-week period. Calls can be coordinated to address multiple areas of non-compliance for a single individual, with outreach prioritized by level of importance. For example, disease management protocols generally supersede preventive care protocols. All notifications to a particular patient automatically stop after the patient books an appointment.

During the notification period, authorized members of your staff have easy access to detailed information about patient compliance status and the reason for each contact, so that they can speak intelligently and effectively with patients who respond to outreach efforts.

Phytel Outreach tracks patient response to notifications and monitors subsequent patient activity. Whenever a contacted patient schedules an appointment, your Patient Registry is updated to display a change in the patient’s compliance status. All communication events are logged, providing details about when patients were contacted, why they were contacted, when they responded, and whether or not the response of a particular patient met the relevant compliance criteria. Failures to respond are also documented for future analysis and planning, and to support your organization’s risk management efforts.
