This can be an extremely labor-intensive process when using traditional claims-based reporting techniques. Phytel can help with a solution that uses patient information already in the Phytel Registry to validate data and automatically complete CMS submission.

An Effortless Submission Process

Phytel’s registry-based reporting simplifies the submission process by allowing providers to report clinical data separately from claims. Eligible providers may report for the 12 month period (January 1st through December 31st) by submitting all measures within a group for 30 patients or by selecting three individual measures for 80% of your Medicare population.

  • Phytel PQRS™ will automatically identify a group of patients based on CMS requirements and the provider’s enrollment selection.
  • Utilizing the patient information already present in the Phytel Registry, medical groups can gather and submit the required PQRS data much more easily and cost effectively than was possible using claims-based reporting.
  • Users can directly enter missing data points into the registry using the web-based entry screen.
  • Phytel then validates the data and completes the submission to CMS.

CMS Qualified Registry 

Phytel’s PQRS™ registry is a CMS qualified, web-based registry that provides practices with a simpler approach to receiving their reimbursement. The registry approach significantly simplifies the reporting process by using clinical data that clinicians are accustomed to and separating the process from the claims submission process. Data can be collected in the registry at any time throughout the year and then is reported as a single transmission to CMS by Phytel at the end of the reporting period.

Measures Supported

Measures Groups are subsets of PQRS measures that have a common focus on a particular clinical condition or aspect of care. Phytel fully supports the following measures groups for 2011:

Group Measures

  • Diabetes Measures Group
  • Preventive Measures Group
  • Chronic Kidney Disease
  • Rheumatoid Arthritis
  • Perioperative

Individual Measures:

As an alternative to measures group reporting, a provider may select three individual measures and must report on 80% of their qualified Medicare patients for each measure.

How to Get Started

Based on each eligible professional’s enrollment selection, Phytel will identify a set of qualifying patients. Users will access this list through the web application and enter any missing clinical data that is required. Once all data elements for at least 30 unique Medicare Part B FFS patients are complete, the user will be prompted to submit the data to Phytel. Phytel will validate the information and send to CMS in the required format.

PQRS 2011 Enrollment Requirements

All existing Phytel Registry clients may enroll in the PQRS offering. The following information will be gathered for each provider wishing to participate:

  • Participating Provider Name
  • Tax Identification Number (TIN)
  • National Provider Identifier (NPI)
  • PQRS Measures Group or Individual Measures Selection
  • Signed Physician Consent Form
